This forms purpose is to protect consumers by limiting agent to discussing just the types of Medicare plans you want to discuss. Signing the form, by typing in your name, does not obligate you in any way. Any ethical agent, who is following Medicare's rules, will send you tout a Scope of Appointment form prior to sending you a plan quote. It's for your proterction!
After reviewing your situation, we will send you out a quote of three plans that we believe offer the best fit for you in taking into consideration your doctor list, prescriptions and your other needs and preferences. The quote will be detailed including estimating your prescription drug and other costs.

First, we will go over your plan choice with you carefully so that you understand how your selected plan works along with all of the benefits and copayment costs to you. Enrollments are usually taken over the phone. We will then track your application through the carrier's approval process and make sure there are no issues or delays.
We will be there for you if any concerns or questions arise once your Medicare plan starts. We are also happy to review plan changes that take place each year during Annual Enrollment Period. At that time each year we can advise you about other plan choices, especially as your needs change.
